Register a NEW STUDENT at JAE  (Kindergarten and students who did not attend JAE last year)

New Student Enrollment Checklist 

 

1.  Register online with Skyward:  

Register a NEW student (click here for help)                             

2. Register your NEW STUDENT at JAE on 7-17-24 from 9:00 AM-12:00 PM.                                                                                

A. Provide copies of 2 Proofs of Residency: (documents should be dated June or July 2024)  Items not accepted: cell phone bill, bank/credit card statements, handwritten agreements 

  • Copy of signed Lease Agreement- Students should be on lease
  • Mortgage Statement- Must provide 2 utility bills with a mortgage statement
  • Electric bill
  • Water bill
  • Gas bill
  • Cable

B.  Print and complete the following documents:

Student Health Information (click here for Spanish form)

McKinney Vento Student Residency Questionnaire (click here for Spanish form)

Jack Anderson Student Enrollment Card

Records Request (Only for those coming from another school)

Home Language Survey  (Only for kindergarteners and students from another country enrolling in a US public school for the first time)

C. Provide the following documents: 

  • Certified Birth Certificate of Student- Mother's copy of birth certificate is not accepted
  • Tennessee Certificate of Immunization Record- If not immunized in Tennessee, immunization record MUST be transferred to a Tennessee form.
  • Physical exam- Completed for kindergarten registration and out of state enrollment
  • Driver's License/state issued ID- may use passport, passport card
  • Custody Papers (if applicable)

Additional Items that may be requested upon enrollment:

  • Proof of Custody/Guardianship- Custody or court ordered parenting plan documents must be provided at the time of registration in cases of legal custodian, shared parneting, or guardianship placements. If a parenting plan is in place, the primary residential parent must register the student in their school of zone.
  • Residency Affidavit- Completed when the custodial/residential parent DOES NOT have a lease or ownership of the residence. Document must be notarized, and 2 proofs of residence must be provided.

4.  MEET YOUR NEW TEACHER!      Attend Back to School Night on 7-29 (Students with last names A-L)  or 7-30 (Students with last names M-Z)                   

Register a RETURNING STUDENT at JAE                 (Students who attended JAE last year)                     

Returning Student Enrollment Checklist

 

1.  Register online with Skyward: 

Register a RETURNING JAE student (click here for help)

2. Register your RETURNING JAE student at JAE on 7-29 or 7-30 from 4:00 pm -7:00 pm at Back to School Night.

A. Provide copies of 2 Proofs of Residency: (documents should be dated June or July 2024)    Items not accepted: cell phone bill, bank/credit card statements, handwritten agreements
