Each parent/guardian of a student in a Sumner County school has the right to refuse the release of their child's name and/or picture for use in the newspaper, on television, on the school's website, or in other such published media reflecting school activities, including athletics.

Signing this refusal form will prevent your child's school form including him/her in recognition for such events as honor rolls, student of the month, classroom or club projects, team experiences, and other school activities.

In the event that I, as parent or legal guardian for the student named below, wish to revoke this refusal, I must notify my child's school in writing

I hereby deny permission for Jack Anderson Elemtary School to publish my child's name and picture in school publications and media releases.
